<p>Researchers from Romania published the results of a study in the <a href="https://www.mdpi.com/1422-0067/25/8/4204"><em>International Journal of Molecular Sciences</em></a> exploring and analyzing the effects of various phytocannabinoids and terpenes working together.</p>
<p>Phytocannabinoids include any naturally occurring cannabinoid found in the plant, from the more commonly known THC, and CBD to less known CBC, CBG, CBN, and more. Researchers wrote that over 100 cannabinoids have been identified in the plant. “The complex interaction between phytocannabinoids and biological systems offers hope for novel treatment approaches and lays the groundwork for further developments in the field of cannabinoid-based medicine,” the <a href="https://www.mdpi.com/1422-0067/25/8/4204">study abstract explained</a>. “This review summarizes the state of the field, points out information gaps, and emphasizes the need for more studies to fully realize the therapeutic potential of phytocannabinoids.”</p>
<p>Researchers explored the entourage effect, which is the combination of both cannabinoids and terpenes working together to create effects that “exceed the sum of their separate contributions.” While consuming a product with a single cannabinoid has its benefits, consuming more than one cannabinoid or terpene together can produce even stronger, more beneficial effects. “This synergy emphasizes how important it is to consider the entire plant when utilizing cannabis medicinally as opposed to just concentrating on individual cannabinoids,” <a href="https://www.mdpi.com/1422-0067/25/8/4204">researchers explained</a>.</p>
<p>Researchers also briefly wrote about terpenes, which make up the aroma and flavor of various cannabis strains. “Standardizing the terpene profile helps maintain consistency in sensory attributes and may also impact the ‘entourage’ effect—the synergistic interaction between cannabinoids and terpenes.”</p>
<p>The study primarily reviewed the known effects and properties of various cannabinoids, many of which offer varying levels of benefit for anti-inflammatory effects, antioxidant effects, pain management, neurological disorders, psychiatric conditions, cancer treatment support, and more. “The exploration of novel phytocannabinoids is rapidly evolving, offering exciting prospects for future therapeutic applications,” <a href="https://www.mdpi.com/1422-0067/25/8/4204">researchers wrote</a>. “Beyond well-established compounds like THC and CBD, the quest for novel cannabinoids widens the scope of potential treatments. Each cannabinoid, with its unique chemical structure, interacts differently with the ECS, suggesting tailored therapeutic effects for specific conditions. This exploration seeks to harness similar benefits while circumventing associated drawbacks.”</p>
<p>The study cited examples of THC having analgesic effects and has been found to help patients treat chronic pain, while CBD has been studied for its anti-inflammatory properties and is an anxiolytic, which has potential to reduce anxiety and stress.</p>
<p>CBG interacts with the body’s endocannabinoid system (specifically the CB1 and CB2 receptors) in a way that suggests it can “modulate the effects of other cannabinoids by influencing CB1 receptor activity.” It has also been studied for its anticancer properties. Similar breakdowns were provided for CBC, CBN, and THCV as well.</p>
<p>This research is the combined presentation from 99 sources to exhibit the benefits of cannabinoids, but spotlights the complex challenges regarding the legal status of cannabis in the U.S. and the EU. However, researchers noted that these laws and regulations are subject to change as more evidence is presented.</p>
<p>Researchers also added that at least in the U.S., more evidence will lead to an expansion in FDA approved products. Currently, only two products—Sativex<sup>®</sup> (which contains both CBD and THC) and Epidiolex<sup>®</sup> (which contains only CBD)—have been approved so far. Sativex<sup>® </sup>was approved in <a href="https://www.clinicaltrialsarena.com/projects/sativex/#:~:text=Regulatory%20approvals%20for%20Sativex&amp;text=Almirall%20submitted%20a%20regulatory%20application,(MHRA)%20in%20June%202010.">2010</a>, while <a href="https://www.dea.gov/press-releases/2018/09/27/fda-approved-drug-epidiolex-placed-schedule-v-controlled-substance-act#:~:text=In%20June%202018%2C%20the%20FDA,years%20of%20age%20and%20older.">Epidiolex<sup>®</sup> was approved in 2018</a>. “CBD and THC should be accessible to patients with special prescriptions due to their potential benefits, especially for individuals with epilepsy refractory to other medications.”</p>
<p>Not all countries in Europe have access to Sativex<sup>®</sup> and/or Epidiolex<sup>®</sup>, which has led to an increase in a reliance on obtaining cannabis products online, and also increases the risk of illegal products that are not being tested for impurities and other harmful contaminants. Due to this, researchers recommended that cannabis products should be made available as a prescription to “lower addiction risk.” “Prescription retention in pharmacies after each refill or release can aid in monitoring usage,” researchers wrote. “Additionally, THC products should adhere to state-specific rules, potentially requiring a special prescription to curb misuse.”</p>
<p>In their conclusion, researchers summarized the many beneficial properties of phytocannabinoids and the need for more research. “By leveraging the synergistic effects of combination therapies and targeting multiple disease pathways, phytocannabinoids hold immense potential to revolutionize the future of pharmacotherapy and improve human health outcomes,” researchers said. “However, further research, regulatory clarity, and standardized protocols are needed to fully unlock their therapeutic benefits and ensure safe and effective use in clinical practice.”</p>
<p>Other researchers have presented similar evidence as well, such as one study published in <a href="https://hightimes.com/study/entourage-effect-not-thc-main-indicator-of-potency-brainwave-study-confirms/">October 2023</a>. A study conducted by Zentrela and PAX showed evidence of the entourage effect (in this case, the application of multiple cannabinoids and terpenes) led to two times the psychoactive effects in comparison to participants who consumed just products with high levels of delta-9 THC.</p>
<p>Similar evidence has been found on mushrooms as well. A study published in <a href="https://www.eurekalert.org/news-releases/1037187">February</a> in the journal Molecular Psychiatry explained how full-spectrum psilocybin extract offers superior benefits compared to synthetic psilocybin. “This research not only underscores the superiority of extracts with diverse compounds but also highlights the feasibility of incorporating them into Western medicine due to the controlled nature of mushroom cultivation,” researchers wrote in a <a href="https://www.eurekalert.org/news-releases/1037187">press release</a>.</p>

<p>A majority of cannabis flower products purchased at licensed retail pot shops in Colorado contained less THC than indicated on the label, according to a study performed by a researcher at the University of Colorado Boulder. The study showed that THC levels indicated on the label of approximately 70% of products tested were at least 15% higher than those reported by a third-party lab after purchase.</p>
<p>The research was conducted by Anna Schwabe, an associate lecture professor of modern cannabis science at the University of Colorado Boulder. To complete the study, Scwabe collected 23 cannabis flower samples from 10 licensed dispensaries located across the northern Colorado Front Range, which includes the cities of Denver, Fort Collins and Garden City. </p>
<p>The samples represented 12 different cannabis strains including sativas, indicas and hybrids, with different THC potency levels reported on the product packaging. Some THC potency levels were reported as a range from 12.8%-19.3% on the lower end and 28.07%-31.28% on the higher limit, while others were reported as a single value such as 16.4% or 17.4%.</p>
<p>After the samples were collected, they were sent to an independent laboratory for analysis using high-performance liquid chromatography (HPLC), which separates, identifies and quantifies the different chemical compounds contained in a mixture. HPLC is the most commonly used method in cannabis testing to determine cannabis potency and detect contaminants, making it an important tool to ensure consumers that products have been tested for safety, purity and potency.</p>
<p>A comparison of lab testing results revealed that approximately 70% of the samples had reported THC levels that were at least 15% higher than the levels determined by the independent laboratory.</p>
<p>“Among the 23 flower samples analyzed, 18 displayed lower THC levels than reported – with 16 falling below 15% of the stated value, 13 falling below 30% of the reported THC and three samples falling below half of the reported THC,” <a href="https://theconversation.com/us/who-we-are">Schwabe wrote</a> in a report about the research published by nonprofit news source The Conversation</p>
<p>Four of the samples were within the range indicated on the label, while only one product contained more THC than the potency level reported on the packaging. </p>
<p>Schwabe noted that the lower THC values determined through independent lab testing were not due to degradation, adding that THC can degrade to the cannabinoid CBN (cannabinol) over time. However, CBN was not detected in measurable amounts in the samples, and additional testing showed that THC levels were stable over time.</p>
<p>The research also revealed a significant increase in <a href="https://hightimes.com/weirdos/the-retail-fallacy-stop-buying-off-thc-percentage/">cannabis potency</a> over the last 15 years, a period that includes the 2014 legalization of recreational cannabis in Colorado.</p>
<p>“THC levels averaged 9.75% back in 2009, based on testing of DEA-seized cannabis flower. Today, levels reportedly surpass 35%, though they’re not as common as consumers have been led to believe,” Schwabe wrote in her report. “DEA-seized cannabis flower averaged 13.88% in 2019, which is closer to my observed mean of 14.98% than the reported mean of my samples, which was 20.27%-24.10%.”</p>
<p>The research did not determine who is responsible for the misinformation found in reported cannabis potency levels. Cannabis cultivators or dispensaries could be selecting the best flowers to submit for laboratory testing. Although regulations require tests to be performed on a random sample of a batch of cannabis flowers, oversight to enforce the guidelines is lacking.</p>
<p>The inaccuracy in cannabis potency labeling could also be the result of fraud. Cannabis testing labs could be manipulating the testing process or doctoring the numbers indicated on certificates of analysis to ensure repeat business or gain new clients, the author of the study noted.</p>
<p>Schwabe noted that accurate lab testing of cannabis potency is important for medical marijuana patients who may want to carefully control their dose of THC. Proper potency testing is also significant for consumers who purchase recreational weed because companies can charge higher prices for cannabis with high levels of THC.</p>
<p>Schwabe, who is also a board member for the nonprofit Agricultural Genomics Foundation, said that the cannabis industry should focus on educating “consumers on how to make more informed choices by looking beyond mere THC percentages.” </p>
<p>“People generally do not shop for wine or beer based on alcohol content,” the researcher wrote. “Instead of focusing on THC content, a novice cannabis consumer might consider brands that are reputable or strains that have an aroma or flavor profile they enjoy. Eventually, they may move on to find a favorite breeder or grower, or a trusted dispensary that employs a knowledgeable budtender.”</p>

<p>Over one-fifth of adults in America reported using hemp-derived cannabinoids such as cannabidiol (CBD) and delta-8 tetrahydrocannabinol (THC) in a survey of responses collected last summer. </p>
<p>Researchers not only identified the extent of how popular hemp-derived products are, but emphasized that states that prohibit cannabis lead to a higher rate of delta-8 THC use—probably because that’s all people can get legally in those states.</p>
<p>The <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2812825">study</a>, “Past-Year Use Prevalence of Cannabidiol, Cannabigerol, Cannabinol, and Δ8-Tetrahydrocannabinol Among US Adults,” was published Dec. 13 in the <em>Journal of the American Medical Association (JAMA) </em>and referenced data gathered in a 2019 Gallup poll.</p>
<p>According to the new findings, 25.2% of adults reported past-year use of any emerging hemp-derived cannabinoid, including delta-8 THC, CBD, cannabigerol (CBG), cannabinol (CBN), etc. Of those, 21% of adults said they had used CBD specifically. Other products derived from hemp had lower rates of use: 11.9% of the survey respondents reported using delta-8 THC, 5.2% reported using CBG, and 4.4% reported using CBN—one of the least significant cannabinoids. </p>
<p>The study was written by doctors Adrianne R. Wilson-Poe, Tristin Smith, Michael R. Elliott, Daniel J. Kruger, and Kevin F. Boehnke. “We provide the first estimates, to our knowledge, of past-year use prevalence of CBN, Δ8-THC, and CBG in the US. A Gallup poll reported that 14% of US adults personally used CBD in 2019; our reported 21% use prevalence of CBD represents a 50% increase over the past 4 years,” researchers wrote. </p>
<p>“Prevalence of past-year cannabis use was somewhat higher than in other studies but was similarly associated with younger age, and past-year cannabis use was also associated with using emerging cannabinoid products,” the report continues. “Higher Δ8-THC use in states without medical or adult-use cannabis laws suggests that cannabis prohibition may unintentionally promote Δ8-THC use. There are few controlled human studies with emerging cannabinoids, but surveys suggest these products are used for treating sleep or pain<a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2812825#zld230228r1"><sup>1</sup></a> and in place of other drugs, including pain medications.”</p>
<p>The researchers noted that hemp products are frequently psychoactive, and these are the main compounds they’re concerned with.</p>
<p>“Based on these results, we support ongoing public health surveillance efforts targeting emerging cannabinoids because of lack of industry standards to protect consumers and similar pharmacology or effects of Δ9-THC and its hemp-derived impairing analogues (eg, Δ8-THC), which may be of particular concern for adolescents and young adults,” the study reports. “Study limitations included not assessing emerging cannabinoid use patterns (eg, dose and use frequency) and possible sampling biases, although NORC implements probability-based recruitment best practices for their AmeriSpeak panel.<sup> </sup>Our results highlight the importance of future research to better understand perceptions of safety, motivations for use, and outcomes of use of these products.”</p>
<p>Researchers cautioned that the products are not regulated and could put patients at the risk of adverse effects from unknown contaminants. </p>
<p>The <a href="https://www.usda.gov/farmbill">2018 Farm Bill</a> legalized the cultivation and sale of hemp, and it created a legal loophole, allowing hemp-derived products that are now sold online and at smoke shops, gas stations, and other retailers. </p>
<p>Medscape reports that the U.S. Food &amp; Drug Administration (FDA) does not regulate hemp-derived products; therefore, no federal standards exist for testing ingredients for safety or verification of ingredients as listed on labels. </p>
<p>“If someone is picking up one of these products to use medically, and it ends up having heavy metals and pesticides and it doesn’t even have the active ingredient in it, that is not good,” <a href="https://www.medscape.com/viewarticle/one-fifth-americans-report-use-hemp-derived-products-2023a1000vqw">said</a> Kevin F. Boehnke, PhD, a research assistant professor in the Department of Anesthesiology and the Chronic Pain and Fatigue Research Center at the University of Michigan in Ann Arbor. </p>
<h2 id="prohibition-leads-to-delta-8-use" class="wp-block-heading"><strong>Prohibition Leads to Delta-8 Use</strong></h2>
<p>In states where medical cannabis is legal, residents had 56% lower odds of using delta-8 THC, whereas those in states where adult-use cannabis is legal had 55% lower odds of using delta-8 THC than those surveyed in states without legalization. </p>
<p>The findings suggest “cannabis prohibition may unintentionally promote delta-8-THC use,” Boehnke and his colleagues wrote. </p>
<p>Alice Kuo, MD, PhD, a pediatrician and professor of internal medicine and pediatrics at the David Geffen School of Medicine at the University of California in Los Angeles, told Medscape that she has seen a large increase in patients who say they use cannabinoid products, and believe them to be “natural.”</p>
<p>“My official response to patients who ask me for my opinion is that I don’t have enough scientific background to give an opinion: If you think it helps you and you have no side effects, then I’m not going to tell you to stop,” Kuo said. </p>
<p>The FDA warns about <a href="https://www.fda.gov/consumers/consumer-updates/5-things-know-about-delta-8-tetrahydrocannabinol-delta-8-thc">delta-8 THC and similar products</a>, which it says are products that have not been proven to be safe.</p>
