<p>A professional organization of physicians who support drug policy reform is calling for the regulation of intoxicating hemp-derived cannabinoids including delta-8 THC, arguing that the safety of products containing the compound is unknown. The recommendation was recently made by Doctors for Drug Policy Reform (D4DPR), a group of healthcare professionals formerly known as Doctors for Cannabis Regulation.</p>
<p>“Our stance at D4DPR is that all intoxicating cannabinoids should be subject to a regulatory framework to ensure public safety,” the group wrote in a <a href="https://www.d4dpr.org/post/d4dprs-recommendations-for-regulating-intoxicating-cannabinoids-chemically-synthesized-from-hemp">policy paper</a> released this month.</p>
<p>In the paper, D4DPR notes that the legalization of hemp with the 2018 Farm Bill “may have inadvertently legalized the chemical conversion of hemp-derived cannabidiol (CBD) and other phytocannabinoids (those derived directly from the plant) into intoxicating minor cannabinoids like delta-8 tetrahydrocannabinol (∆8-THC, also known as delta-8).” The legislation, however, did not include provisions to regulate hemp-derived cannabinoids, leading to a thriving industry of products containing intoxicating compounds that is unregulated in many jurisdictions.</p>
<p>“Taking advantage of this opportunity, ∆8-THC (chemically synthesized from hemp CBD) quickly became available in various retail outlets such as gas stations, CBD shops, convenience stores, smoke shops, and online platforms,” the group notes in the paper. “Several states have now either banned or imposed regulations on its sale. However, in 22 states (as of November 2023), ∆8-THC remains legal and unregulated, with limited laboratory testing and taxation, lacking warnings about its intoxicating effects, without dosing limits, and easily accessible to minors.”</p>
<p>To address the issue, D4DPR called on policymakers to develop and enact a regulatory framework for all intoxicating cannabinoids, regardless of their source. The group included several recommendations for the regulations, including a provision that would only allow the sale of intoxicating compounds by licensed dispensaries. The group also called for “appropriate taxation” to fund public health initiatives and regulatory oversight of the cannabinoid market.</p>
<p>The group also recommended that sales of intoxicating cannabinoids be restricted to adults aged 21 and older. The recommendations call for intoxicating hemp products to be sold only in child-resistant packaging that does not appeal to minors, with clear labeling about the intoxicating effects of the product. Packages should also include the International Intoxication Cannabinoid Product Symbol (the silhouette of a cannabis leaf) to indicate their contents in graphic form.</p>
<p>The recommendations also call for required lab testing of intoxicating cannabinoid products for purity, potency and safety, with certificates of analysis available to consumers for inspection. The group also recommended that research into the clinical safety and toxicology of minor cannabinoids be conducted, noting that many of the compounds are new to the market </p>
<p>D4DPR also recommended that regulations for intoxicating cannabinoids be aligned with those in place in states with medical marijuana or adult-use cannabis programs and that states without regulations develop them as soon as possible. The group also called for the rescheduling of cannabis at the federal level, noting that a ban on intoxicating or minor cannabinoids “will result in a continuation of the drug war, leading to negative outcomes on public health.”</p>
<p>“This policy stance reflects our commitment to safeguarding public health while ensuring reasonable access to cannabis- and hemp-derived products within a responsible regulatory framework,” the D4DPR concluded in its recent policy paper.</p>
<p>The policy paper from D4DPR joins a chorus of calls to regulate intoxicating cannabinoids. Last month, 21 state attorneys general <a href="https://hightimes.com/news/state-attorneys-general-ask-congress-to-regulate-intoxicating-hemp-products/">sent a letter to congressional leaders</a> asking them “to address the glaring vagueness created in the 2018 Farm Bill.” </p>
<p>“The reality is that this law has unleashed on our states a flood of products that are nothing less than a more potent form of cannabis, often in candy form that is made attractive to youth and children — with staggering levels of potency, no regulation, no oversight, and a limited capability for our offices to rein them in,” <a href="https://oag.ca.gov/system/files/attachments/press-docs/Corrected%20-%20Ltr%20Concerning%20Five-Year%20Farm%20Bill%20Reauthorization.pdf">they wrote</a> in the letter.</p>

<p>The Senate Appropriations Committee <a href="https://www.appropriations.senate.gov/hearings/full-committee-markup">approved a spending bill</a> last Thursday that includes an amendment allowing the <a href="https://hightimes.com/news/study-1-in-10-us-veterans-used-cannabis-in-past-year/">U.S. Department of Veterans Affairs</a> (VA) doctors to recommend medical cannabis for their patients in legal states. It will now move forward as part of the approved legislation that funds the VA for the 2024 Fiscal Year.</p>
<p>The amendment, which passed via a voice vote, was sponsored by Senator Jeff Merkley, a Democrat from Oregon. It will lead to the same results desired in a <a href="https://hightimes.com/news/house-lawmakers-reintroduce-bipartisan-veterans-equal-access-act/">standalone bill refiled in the House</a> with bipartisan backing by Representative Earl Blumenauer, a Democrat from Oregon, and Florida Republican Representative Brian Mast, who lost both legs while serving in the Army in Afghanistan. Collectively they are the co-chairs of the Congressional Cannabis Caucus. </p>
<p><a href="https://www.appropriations.senate.gov/hearings/full-committee-markup">The more recent measure</a> that just passed in The Senate Appropriations Committee “simply says, in states that have a medical cannabis program, that a veteran’s doctor can talk to their veteran patient about the pros and cons of medical cannabis and fill out related paperwork should a veteran decide to participate in a state program where such paperwork is required,” Merkley said. </p>
<p>The amendment yields the same outcome as The Veterans Equal Access Act, which has not yet been implemented despite passing in committees and clearing floor approval multiple times with bipartisan (not to mention veteran) support. </p>
<p>In December of 2022, a coalition of more than 20 veterans service organizations (VSOs) wrote a letter to congressional leaders that just about had to beg lawmakers to pass a cannabis and veterans research bill before the end of the previous Congress session. </p>
<p>“For decades, many veterans have called for medicinal cannabis as an option for treating the unseen wounds of war and other injuries sustained through service,” the letter reads. “Veterans and caregivers have consistently communicated their anecdotal experiences regarding how cannabis offers effective treatment in tackling some of the most pressing health concerns they face upon returning from war.” Given what vets give for this country, and Americans now say that <a href="https://hightimes.com/news/americans-say-cannabis-is-safer-than-alcohol-and-cigarettes-and-less-addictive-than-technology/">cannabis is safer than alcohol and cigarettes</a>, it is more than a fair ask. </p>
<p>No such legislation passed in time. However, thanks to the approval of the latest spending bill, vets can now talk to their doctors about medical cannabis. Research continues to show the valuable role cannabis can play in treating <a href="https://hightimes.com/study/new-study-shows-evidence-that-low-dose-thc-treatment-could-help-treat-veteran-ptsd/">PTSD</a>, depression, anxiety, and many other conditions that, unfortunately, are all too familiar among vets. </p>
<p>Late last year, a study published in the journal <a href="https://www.sciencedirect.com/science/article/abs/pii/S0028390822002817#!"><em>Neuropharmacology</em></a> by researchers from Wayne State University showed evidence that low doses of THC help treat adults with PTSD. While there is plenty of previous research on cannabis and trauma, this was the first to explore how THC affects corticolimbic brain activation. </p>
<p>Additionally, for those vets with injuries or chronic pain, cannabis offers a safer and harm-reduction path to treating such pain and allows many vets to opt out of opiates. </p>
<p>“We remain committed to the VA’s goal of conducting research into the efficacy of medicinal cannabis as a treatment for veterans with chronic pain, PTSD, and Traumatic Brain Injuries,” their letter continues. “However, as a Schedule I drug under the [Food and Drug Administration], research into the efficacy of cannabis has been stagnant, cumbersome, and convoluted with red tape. Federal research into cannabis faces many bureaucratic hurdles that hinder researchers.”</p>
<p>In related news, Representative Mariannette Miller-Meeks, a Republican from Iowa, filed different legislation in May that would promote research for treating conditions such as PTSD and chronic pain within the VA, and that’s just one of many bills currently making their way through Congress. </p>
<p>In April, bipartisan House representatives and Senators also refiled bills to legalize medical marijuana for military veterans. If made into law, it would allow veterans to legally possess and use cannabis on a federal level (but following state law) as recommended by their doctor.</p>

<p>Due to an uptick of incidents involving seniors consuming too much cannabis, doctors are warning older citizens about how to consume the plant correctly. Eating edibles in particular, without the proper guidance, is <a href="https://www.quora.com/Is-it-normal-to-get-a-panic-attack-from-weed-edibles">notorious</a> for causing panic, though the risks are rarely physical.</p>
<p>KOMO News in Seattle <a href="https://komonews.com/news/local/marijuana-pot-cannabis-weed-mixing-seniors-seattle-doctors-warn-increase-emergency-room-visits-for-elderly-people-potency-ed">reports</a> that doctors want to inform seniors about ways to avoid panic and an unnecessary emergency room visit. This can be solved with simple education about the differences between edibles, topicals, and smokable products and how they affect us differently.</p>
<p>It’s the responsibility of any consumer to properly research before eating an edible, or consuming a high-THC product.</p>
<p>“There are a number of different situations where you could end up in the emergency department, because of a combination of cannabis and a medication or cannabis and another product,” Dr. Lianne Hirano of the Geriatrics department at Harborview Medical Center in Seattle, Washington, told KOMO News. </p>
<p>“If you are a first time user, or if you are somebody who is used to using products from the 60s and 70s, today’s products are way more potent. Your body and your metabolism might not be used to that level of potency,” she said, “That can be quite scary.”</p>
<p><em>Wall Street Journal</em> <a href="https://www.wsj.com/articles/marijuana-cannabis-edibles-pot-cbd-health-risks-for-older-people-seniors-67d3226c">reports</a> that seniors are often susceptible to consuming too much cannabis if they’re used to lighter doses.</p>
<p>There are a number of reasons seniors are trying out cannabis for medical purposes. Some seniors <a href="https://hightimes.com/news/cannabis-use-associated-better-sleep-seniors/">turn to cannabis to help with sleeping</a>. Researchers found that overall, cannabis helped seniors get, on average, an extra 30 minutes of sleep. Study participants used smartphones and actigraphy watches to log their sleep patterns</p>
<p>Some seniors believe <a href="https://hightimes.com/news/study-finds-seniors-use-cannabis-treat-symptoms-aging/">cannabis can help when battling aging itself</a>. Seniors are turning to cannabis to treat common symptoms of aging, with nearly 80% of those who reported using cannabis saying they did so for medicinal reasons, according to a study from researchers at the University of California San Diego. Results of the study, “Cannabis: An emerging treatment for common symptoms in older adults,” were <a href="https://health.ucsd.edu/news/press-releases/2020-10-07-study-finds-older-adults-using-cannabis-to-treat-common-health-conditions/">published</a> on Oct. 7, 2020 in the J<em>ournal of the American Geriatric Society</em>.</p>
<h2 id="todays-weed-is-a-different-animal"><strong>Today’s Weed is ‘A Different Animal’</strong></h2>
<p>Hashtag Cannabis Marketing Director Christine Bryant described today’s more potent weed as “a different animal.”</p>
<p>“The difference is when using a topical, you’re not going to feel the intoxicating effects of THC. But you will feel some of the anti-inflammatory effects that THC can provide for the skin barrier. That’s the difference with an oral (product), like a gummy or a chocolate, something like that. Is this going to affect your whole system? You’re going to process it through your liver, you’re going to feel whole body effects,” she said.</p>
<p>In the event that you consume too much THC, <a href="https://cornerstonecollective.com/how-to-counteract-a-too-intense-cannabis-high/#:~:text=The%20Black%20Pepper%20Trick,can%20subdue%20feelings%20of%20paranoia.">sniffing black peppercorns</a> is an old hack that can reduce some, but not all, of the effect.</p>
<p>Studies show both improvements and setbacks due to senior cannabis use. Last January, researchers from the University of California San Diego published a <a href="https://agsjournals.onlinelibrary.wiley.com/doi/abs/10.1111/jgs.18180">study</a> in the <em>Journal of American Geriatrics Society</em>, suggesting that emergency room visits involving people 65 and older who use cannabis have increased 1,804% in the past 15 years.</p>
<p>Another study published in the <a href="https://agsjournals.onlinelibrary.wiley.com/doi/abs/10.1111/jgs.16833"><em>Journal of American Geriatrics Society</em></a> also reported an uptick in seniors who are consuming cannabis. In a survey of nearly 600 adults in the age range found that 15% reported using marijuana products in the last three years.</p>
<p>Anyone getting into cannabis should be aware of the proper titration in order to prevent panicked calls to the emergency room.</p>

<p>If Thailand’s transformation from a nation with one of the strictest set of drug laws to a place with legal cannabis seemed like <a href="https://hightimes.com/news/cannabis-legalization-criticized-in-thailand-as-too-much-too-soon/">too much, too soon</a>—you’re not alone. Doctors from across Thailand are speaking out on what they consider to be a reckless cannabis law that lacks protection against children and teens.</p>
<p>President of the Forensic Physician Association of Thailand, Smith Srisont, posted a document on <em>Change.org</em> in Thai with a list of demands to the government. “Turn off the vacuum state of cannabis” the post reads. The “vacuum” is the way the document describes the lack of regulatory controls compared to systems seen in place in other countries. Srisont helped determine the <a href="https://pubmed.ncbi.nlm.nih.gov/34054017/">dangers of other substances like kratom</a> in the past.</p>
<p>Over 851 doctors signed the petition, which was posted in Thai, but it should be noted that all 851 signees are doctors and alumni of one single school—the Faculty of Medicine Ramathibodi Hospital, Mahidol University in Ratchathewi, Bangkok. However, the school is considered the country’s most respected medical establishment.</p>
<p>The document states that studies show that cannabis could affect the health of children and teenagers as well as the growth of their brains. The document added that with legalization on the agenda, the move now threatens the healthcare system and Thai people’s health, both in the short- and long-term.</p>
<p>“Cannabis was removed from the Public Health Ministry’s Narcotic list on June 9, but no policies have been launched to control the use of cannabis for personal pleasure,” a spokesperson from the group <a href="https://thethaiger.com/news/national/leading-academics-doctors-in-thailand-demand-govt-to-pause-cannabis-bill">stated</a>. This lack of [legal] direction makes cannabis more accessible for children and teenagers.”</p>
<p>Thailand’s Public Health Minister Anutin Charnvirakul was accused Tuesday, by the Opposition party, of deliberately creating a “legal vacuum” in Thailand, in which there is no law to enable control of the use of cannabis, hemp, or extracts following decriminalization of cannabis in the country last month.</p>
<h3 id="fear-in-thai-media"><strong>Fear in Thai Media</strong></h3>
<p>The country also experienced a wave of fear-inducing news articles following the rollout of legal cannabis in the country. <em>Thai PBS</em> <a href="https://www.thaipbsworld.com/will-cannabis-legalization-without-regulation-come-back-to-burn-thailand/">reports</a> that in the month following legalization, nine children fell ill from unsupervised cannabis consumption. It was also reported that a 6-year-old child ate cannabis-infused snacks and “fell into a stupor,” while a 15-year-old boy with a known history of depression attacked people with a knife after smoking two joints.</p>

The target of the campaign was set at 15,000 names, and the campaign got more than 10,800 people.</p>
<p>“The government and related departments should stop threatening people’s health as soon as possible,” the document reads at the end. “The use of cannabis for medical purposes should be under control for the best benefits and safety as the government claimed from the first place.”</p>
<p>Thai doctors began raising concerns that cannabis supposedly can trigger mental health issues months ago. Criticism is also coming from people who are concerned that the change in the law will hurt the reputation of Thai <a href="https://hightimes.com/news/shining-a-spotlight-on-the-thailand-cannabis-community/">agricultural exports</a>, given that the biomass might be used in animal feed.</p>
<p>Some of the changes have been too much to handle for people in the county. On the bright side, it’s having a lasting positive impact as well. Thailand’s sudden change of heart towards cannabis is prompting other countries in the region such as Indonesia to re-examine their own approach to cannabis.</p>
