<p>Delta-9-THC (delta-9-tetrahydrocannabinol) is also one of the most researched cannabinoids for pain. For example, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2503660/#b138">this medical review</a>  looked at many experiments and concluded that “THC has twenty times the anti-inflammatory potency of aspirin and twice that of hydrocortisone” without the bleeding risks associated with all nonsteroidal anti-inflammatory drugs (NSAIDS).</p>
<p>Although CBG oil (cannabigerol) is less well known than CBD and THC, CBG may have even more pain-relieving properties than THC. <a href="https://pubmed.ncbi.nlm.nih.gov/28348416/">This scientific study</a> found “CBG has more potent analgesic (pain-relieving), anti-erythema (anti-inflammatory rash) and lipoxygenase (inflammation) blocking activity than THC. Meanwhile, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9210160/">this scientific study</a> found that CBG may work by blocking the transmission of pain signals.</p>
<p>While each cannabinoid – THC, CBG and CBD – offers relief on its own, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8627890/">this chronic pain study </a>found that “THC + CBD + CBG combined was more effective than these compounds applied alone.” The researchers said results highlighted the potential of using these three specific cannabinoids, particularly in combination.</p>

<p>A new <a href="https://journals.lww.com/pain/abstract/9900/terpenes_from_cannabis_sativa_induce.589.aspx">study</a> released earlier this month on May 2 in the research journal <em>Pain</em> suggests that an injection of cannabis terpenes is “roughly equal” to the number of pain reduction markers from an injection of morphine.</p>
<p>“<a href="https://journals.lww.com/pain/abstract/9900/terpenes_from_cannabis_sativa_induce.589.aspx">Terpenes from Cannabis sativa induce antinociception in a mouse model of chronic neuropathic pain via activation of adenosine A2A receptors</a>” features researchers hailing from multiple departments at the University of Arizona as well as with awarded funding from the National Institutes of Health.</p>
<p>Researchers sought out to see how cannabis can potentially treat chronic pain. “A question that we’ve been very interested in is could terpenes be used to manage chronic pain?” <a href="https://healthsciences.arizona.edu/news/releases/study-shows-cannabis-terpenes-may-relieve-chemotherapy-induced-neuropathic-pain#:~:text=A%20new%20University%20of%20Arizona,relief%20without%20negative%20side%20effects.">said John Streicher, PhD</a>, who is the lead author of the study. “What we found is that terpenes are really good at relieving a specific type of chronic pain with side effects that are low and manageable.”</p>
<p>While both CBD and THC have been found to be beneficial in treating chronic pain, researchers explained that the effects are moderate, and “can come with unwanted psychoactive side effects.” Terpenes are a path around the use of psychoactive substances, which “offer an alternative path to pain relief without adverse side effects.”</p>
<p>For the purpose of this study, researchers examined five different terpenes: alpha-humulene, beta-caryophyllene, beta-pinene, geraniol, and linalool. Researchers utilized a mouse model focused on neuropathic pain, which is a well-known condition that stems from nerve damage caused by chemotherapy medications.</p>
<p>Each terpene was tested on its own and the results were compared with morphine as well. All five terpenes showed significant pain reduction “near to or above the peak effect of morphine” individually, and even more powerful effects on pain when combined. “That was really striking to us, but just because something relieves pain doesn’t necessarily mean it’s going to be a good therapy,” Streicher said.</p>
<p>The negative effects of using opioids include an activation of the reward system in the brain, which can lead to an increased tolerance to the effects over time, and addiction as well. But terpenes don’t lead to these same effects. “We looked at other aspects of the terpenes, such as does this cause reward? Is this going to be addictive? Is it going to make you feel awful?” said Streicher. “What we found was yes, terpenes do relieve pain, and they also have a pretty good side effect profile.”</p>
<p>Researchers also examined the method of consumption with terpenes, including injection, oral dosing, and inhalation. When orally administered or inhaled, the effects were “significantly reduced or absent.” “A lot of people vape or smoke terpenes as part of cannabis extracts that are available commercially in states where cannabis use is legal,” said Streicher. “We were surprised to find that the inhalation route didn’t have an impact in this study, because there are a lot of at least anecdotal reports saying that you can get the effects of terpenes whether taken orally or inhaled. Part of the confounding factor is that terpenes smell quite nice and it’s hard to disguise that aroma, so people could be kind of having the psychosomatic placebo-style effect.”</p>
<p>Previously, Streicher and his team published their findings from a similar study back in <a href="https://healthsciences.arizona.edu/news/releases/study-shows-cannabis-terpenes-provide-pain-relief-contribute-entourage-effect">July 2021</a>. At the time, their results showed promising evidence regarding terpenes being used for pain relief, especially when studying the benefits of the entourage effect. Although their hypothesis was to examine how cannabinoids affected pain, they were ultimately surprised by how powerful the entourage effect actually was. “It was unexpected, in a way,” <a href="https://healthsciences.arizona.edu/news/releases/study-shows-cannabis-terpenes-provide-pain-relief-contribute-entourage-effect">Streicher said in 2021</a>. “It was our initial hypothesis, but we didn’t necessarily expect terpenes, these simple compounds that are found in multiple plants, to produce cannabinoid-like effects.”</p>
<p>Looking toward the future, Streicher plans to utilize these results in his next research endeavor. In a press release, he explained that his next hypothesis will explore if terpenes can block the body’s reward system and also still provide the same pain treatment as seen in the most recent results. “This brings up the idea that you could have a combination therapy, an opioid with a high level of terpene, that could actually make the pain relief better while blocking the addiction potential of opioids. That’s what we are looking at now,” said Streicher.</p>
<p>Over time, other terpene studies have revealed how they can serve as a treatment option for certain conditions. </p>
<p>A study from 2020 published in <a href="https://hightimes.com/study/study-terpene-levels-in-cannabis-affect-patient-outcomes/"><em>Molecules</em></a> defined terpenes in detail to “describe sources of cannabis terpenes and to explain the biosynthesis and diversity of terpene profiles in different cannabis chemovars.” Their results showed that terpenes and cannabinoids used together show evidence of the entourage effect.</p>
<p>In February 2023, one study from researchers at the University of New Mexico found that patient relief was best experienced with higher terpene levels.</p>
<p>In October 2023, a study performed by researchers at Abstrax Tech and published in the American Chemical Society’s journal, <a href="https://pubs.acs.org/doi/10.1021/acsomega.3c04496"><em>Omega</em></a>, showed that the importance of terpenes is “overstated” when it comes to defining the differences between cultivars. “This discrepancy suggests that while these classifications may be helpful for chemotaxonomic purposes, they lack the chemical information necessary to differentiate these varieties from an aroma perspective,” <a href="https://pubs.acs.org/doi/10.1021/acsomega.3c04496">the study stated</a>.</p>

<p>Tryp Therapeutics <a href="https://www.accesswire.com/756926/Tryp-Therapeutics-Submits-Investigational-New-Drug-IND-Application-to-US-FDA-for-Planned-Phase-2a-Clinical-Trial-in-Patients-With-IBS-at-Massachusetts-General-Hospital">announced</a> on Wednesday that it had “submitted an Investigational New Drug (IND) application to the U.S. Food and Drug Administration (FDA) for its planned Phase 2a clinical trial investigating the effects of psilocybin-assisted psychotherapy in the treatment of patients aged 21+ suffering from Irritable Bowel Syndrome (IBS) at Massachusetts General Hospital (MGH).”</p>
<p>The Canadian company said in the announcement that the “planned open label study in collaboration with Harvard Medical School/Massachusetts General Hospital will evaluate the effect of psilocybin-assisted psychotherapy in patients with treatment-resistant IBS who experience chronic abdominal pain and other debilitating gastrointestinal symptoms.” </p>
<p>“Many of these patients also suffer from fibromyalgia, anxiety and fatigue. The primary efficacy endpoint of the study will be improvement in abdominal pain. The proposed study will also explore changes in brain connectivity and responses to pain at baseline and at four weeks, six months and twelve months post the psychedelic drug sessions, along with numerous other secondary endpoints,” the announcement said. </p>
<p>“Tryp and our collaborators at Harvard/MGH believe there is tremendous potential for the treatment of debilitating IBS symptoms by utilizing the combined administration of psilocybin and psychotherapy. The clinical study will examine how psilocybin-assisted psychotherapy may alter brain networks involved in chronic abdominal pain and gastrointestinal-specific anxiety in patients with IBS to improve their symptoms. Submission of IND 163994 is an important step in advancing our program,” said Jim Gilligan, the chief executive officer of Tryp Therapeutics.</p>
<p>Gilligan told <a href="https://www.greenmarketreport.com/tryp-therapeutics-advances-psilocybin-therapy-for-ibs-with-fda-ind-submission/">Green Market Report</a> that the “most important thing is a clinical data – to be able to not just assume or hypothesize that we’re going to have a benefit, but to actually demonstrate that we can do something positive for patients.</p>
<p>“We’re looking at things a little bit differently than the big guys, looking at unique areas where we can have first-mover advantage. But we’re judicious in selecting areas where we really think that we’ll have a positive outcome,” Gilligan said.</p>
<p><a href="https://www.greenmarketreport.com/tryp-therapeutics-advances-psilocybin-therapy-for-ibs-with-fda-ind-submission/">According to Green Market Report,</a> Gilligan “likened the planned administration of psilocin to the work of anesthesiologists.”</p>
<p>“Using an IV to induce and subsequently awaken the patient from the psychedelic state, the approach might also allow for the use of serotonin antagonists to terminate the psychedelic experience, if necessary,” the outlet <a href="https://www.greenmarketreport.com/tryp-therapeutics-advances-psilocybin-therapy-for-ibs-with-fda-ind-submission/">said</a>, which noted that TRP-8803 will be “central” to the company’s approach to the therapy.</p>
<p>TRP-8803 is “Tryp’s lead program,” the company says, describing it as “a proprietary formulation of IV-infused psilocin (the active metabolite of psilocybin) that alleviates numerous shortcomings of oral psilocybin including: significantly reducing the time to onset of the psychedelic state, controlling the depth and duration of the psychedelic experience, and reducing the overall duration of the intervention to a commercially feasible timeframe.</p>
<p>“The Company has an ongoing Phase 2a clinical trial for the treatment of Binge Eating Disorder at the University of Florida, an upcoming Phase 2a clinical trial with the University of Michigan for the treatment of fibromyalgia and a planned Phase 2a trial for the treatment of irritable bowel syndrome at Mass General Hospital, all of which are utilizing TRP-8802 (synthetic, oral psilocybin) to demonstrate efficacy in these indications. Where a preliminary clinical benefit has been demonstrated, subsequent studies are expected to utilize TRP-8803 (IV-infused psilocin) which has the potential to further improve efficacy, safety and patient experience,” Tryp said in Wednesday’s announcement. </p>

<p>A new report from <a href="https://hightimes.com/news/new-frontier-data-explores-how-packaging-affects-consumer-purchasing-habits/">New Frontier Data</a> explores the necessity of understanding what medical cannabis patients are currently looking for and require in cannabis medicine. Entitled “<a href="https://info.newfrontierdata.com/from-doctor-to-dispensary">From Doctor to Dispensary: A Complete Picture of Medical Cannabis Consumers</a>,” the report analyzes data from a recently published <a href="https://hightimes.com/environment/new-frontier-data-report-identifies-where-consumers-like-to-smoke/">consumer survey</a>, which includes input from 1,874 medical cannabis patients.</p>
<p>New Frontier Data argues that insurance companies could benefit the most from better understanding medical patients. “The Federal Schedule 1 status of cannabis means that it has no recognized medical value. At the same time, almost every state in the U.S. allows for some form of medical cannabis use,” <a href="https://newfrontierdata.com/cannabis-insights/providing-a-prescription-for-meeting-the-needs-of-medical-cannabis-patients/">the report states</a>. “This contradiction has resulted in millions of medical cannabis patients with the approval to use cannabis by their state and their doctor, but no access to insurance reimbursement for this medicine, which is usually highly taxed.”</p>
<p>Among the most common conditions that medical patients treat with cannabis is <a href="https://newfrontierdata.com/cannabis-insights/providing-a-prescription-for-meeting-the-needs-of-medical-cannabis-patients/">pain</a> (47% of surveyed participants), followed by anxiety/panic attacks (22%), depression (9%), sleep disorders (9%), PTSD (7%), neurological disorders (6%). Additionally, 93% of medical cannabis consumers say that cannabis has helped them, with 57% saying that it significantly improved their medical conditions (while only 36% say that it only slightly improved their condition).</p>
<p>Fifty percent of patients reported that they were currently using prescription drugs, mainly antidepressants (45%), muscle relaxers (27%), arthritis (23%), sleep (20%), and opioids for pain (17%). Over half of the patients also said that they have replaced some or all of their prescription medications with cannabis as well.</p>
<p>The federally illegal status of cannabis is a roadblock for many parts of the industry, but opinions about cannabis are rapidly changing. “This information is especially useful to insurance companies who might view cannabis as a medicine with less potential harm and therefore less cost to them than other medications,” <a href="https://newfrontierdata.com/cannabis-insights/providing-a-prescription-for-meeting-the-needs-of-medical-cannabis-patients/">the report explained</a>. “Less harm may also be a driver for the medical cannabis consumer as only 40% say that saving money is a reason for the substitution.”</p>
<p>New Frontier Data also advises how brands can best cater to medical cannabis patients versus average recreational consumers. Both medical and recreational consumers prefer to consume cannabis on their own, or without others. In products with a hybrid of cannabinoid content, only 43% of medical consumers chose products with more THC than CBD, compared to 57% of recreational consumers. However, medical cannabis consumers led with 54% for products that had “slightly more CBD than THC,” and 57% with products that contained “much more CBD than THC.”</p>
<p>Overall, the report found that 55% of medical consumers believe it’s very important to consider minor cannabinoids or terpenes when making a purchase (with 23% saying “I don’t know what that is.”)</p>
<p>Retailers also stand to update their approach to serving medical cannabis patients, as they usually have strong loyalty to strains that they know are effective. Fifty-nine percent usually or always pick the same strain, while only 34% will alternate between options they are familiar with. Most consumers, both medical (52%) and recreational (50%), tend to spend around $50-$200 on cannabis products per month, while a lower percentage will spend more than $200 (30% for medical and 33% for recreational).</p>
<p>“With all the focus on adult use legalization and the recreational consumer, it can be easy to forget that there is a large population of consumers who rely on cannabis for their physical and mental health,” <a href="https://newfrontierdata.com/cannabis-insights/providing-a-prescription-for-meeting-the-needs-of-medical-cannabis-patients/">the report concluded</a>. “And while some may argue that all cannabis use is medical, there are certainly distinguishing factors for medical consumers as evidenced above. As cannabis continues its march towards federal medical acceptance, insurance companies, brands and retailers should be aware of the needs of this group and how to meet them.”</p>
